As a Zeel Provider, your ability to evaluate each patient and document their condition is essential to delivering high-quality care. The SOAP documentation format allows you to provide clear, structured notes that support better patient outcomes.

Tips for Efficient & Effective Documentation

Complete Notes Promptly

  • Document as soon as possible after treatment—ideally immediately after the session while details are fresh in your mind.

Keep It Simple & Time-Efficient

  • First & Fifth Appointments include a GPS assessment that can take up to 30 minutes

  • All other Appointments include a DVPRS pain assessment

Use Voice Dictation (With Caution)

  • You can use your phone’s talk-to-text feature in the Zeel Provider App.

  • 🚨 Always proofread before submitting—voice dictation is not always accurate.

Do NOT Copy Answers from previous SOAP Notes

  • Each session must have unique documentation reflecting the patient’s progress and response to treatment.

Review & Proofread Before Submitting

  • SOAP notes should be treated as professional documents.

  • Check for clarity, spelling, and accuracy to ensure high-quality documentation.

  • You are able to edit the subject, objective, and treatment section before checkout. After checking out, you can add/edit before submitting the Assessment and Plan section of the SOAP note.
