Certificate of Insurance Requirements

When submitting your updated massage therapy Certificate of Insurance please verify that the document uploaded meets the necessary criteria.

• Name on COI must match name on your Zeel provider account and/or legal name.

• At least $3,000,000 annual aggregate coverage.

• At least $1,000,000 per occurrence limit.

• Full COI document is uploaded.

  • Insurance cards, renewal receipts and similar are not accepted. See examples.

  • Your full COI is available for download on your insurance provider's website and was likely emailed to you when your policy was renewed/created.

In order to upload your COI please select "My Profile" from the sidebar menu of your provider app, select "Credentials" and follow the on-screen update prompts!
