Clinical Documentation Requirements

• On the first visit, providers must complete a comprehensive initial evaluation.

• For every treatment, providers must complete:

  • Functional outcome & pain scales

  • Detailed SOAP notes

• Do not share patient information with anyone outside of Zeel’s clinical team.

  • Violating HIPAA can result in suspension of your professional license.

• If you need to communicate with Zeel about a medical treatment:

  • Use secure, in-app chat.

  • Never email or text about medical appointments or patients.
